Filling up RPRF receipt.

Hello. Can anyone tell me whose information do I put in client DoB, address telephone number etc in the RPRF Receipt. My mother paid for the fee and her name is automatically encoded in the PDF. Im confused. Thank you!

We were advised to put the name and address of the person who paid on the receipt.

Do you already have a case number? Or UCI? If so, fill in the DOB of the principal applicant and if you have a UCI put that on the form too.

I'm sure Seniors will correct me if I'm wrong.
