Filling out Statutory Declaration of Common Law Union

Hello guys,

I got a few questions regarding the declaration of common law union. Both my girlfriend and me are Croatian citizens living in Germany. Our common law partnership started when we moved to Germany and started living together. Since my last application got rejected because I forgot to add this very document, I want to do it right this time.

1. At the very beginning of the document it says: If applicable and then country and province. Do we have to put something there?

2. Secondly, just a bit more down the document asks again for the country, do we put Germany or Croatia there? In the same part there is an ending date, do we put "ongoing" because our relationship is still lasting?

3. I suppose at the part where we have to fill out the solemn declaration at the very bottom, we put the country where we are signing the document, in this case Germany?

4. Lastly, my girlfriend is the one paying rent to our landlord from her bank account. I am sending half of the rent to her bank account every month. Since we haven't signed a lease together, I assume I have to explain the situation under number 4?

1- Provide the German address
2-Germany, ongoing
3-Yes
4-It's enough to prove that you are living under the same address, you can use various documents showing you both have had the same address for at least 12 months
