Hi! I'm a permanent resident applying to sponsor my husband from the Philippines (outland application)

Just a few questions regarding the forms:

1.) We were married in February 2011 but I have not changed my surname (to my husband's surname) in all my identification cards. Is it okay to use my maiden name in all the application forms?

2) The documents ask for a last name and given names. I have 2 given names, a middle name(my mother's maiden name) and my surname. Should I just put everything down? My passport has all of my names, but my pr card has my 2 given names and my surname only.

3) I filled out all the documents using CAPITAL BLOCK letters. Is that okay or should i re-write everything using capital letters only when needed?

I'm almost done with all the paperwork and i just wanted to be thorough before i submit them this week. I'm really confused ???, i know some of them are only minor questions but i don't want to make any mistakes and have them return my app.

1) Use the name that matches your ID, yes.

2) Put down your full name. Chances are your PR card only has given and surname because there wasn't enough space.

3) Block CAPS is fine, the form just needs to be readable.
