I have sent my application for processing to CIC malaysia, after they asked me to submit all document within 120 days. I have already been given one file number BXXXXXXXX, will they going to give me new file number before they process my complete application or will i get medical request if my case is successful..?

No, its not 4 months for everybody. it depends upon visa office. New delhi the current time is 45 to 60 days after sumbitting final documents as per the current trend.

I received my new number in 10 days after it was delivered to them. But I think new number doesn't mean that your application is gonna go through.

The letter said:

"... We have created a file with the file number pasted above. Please note that this file number is different than the one that was assigned to you by the Case Processing Centre in Sydney, Nova Scotia, Canada. You must quote this file number in any future correspondence.

Before your application can be placed in processing, a determination will be made as to whether or not you are eligible for processing. If your application is not eligible for processing, you will receive a letter informing you of the reasons for the decision as well as a refund..."

I sent my initial application to CIC malaysia and final to them as well, what i feel is that anyone sending application to nova scotia and than to local offical get two file numbers. Is this right or even i send my both application initial and final to local CIC will get two file numbers.

I guess, every file number shows which visa office your case belongs to. They change it if your application is transfered to another visa office.
