I dont use my family name and same it is
not passport also I have given name only
so leave this field blank or put N/A

Your residencial address if diff
writing -same- is ok ?


Fee payment form I am the one who is
principal applicant and i m paying from
my credit card

it says write name if payer is diff.

should I write my name again or put N/A

and same for address column if payer is diff. then
write address


background declaration form

Q 7 my parents are alive so date of death

put N / A by pen or leave it

Leave it and write with Pen if not able to write NA

Better if you fill in Capital letters

Check for answers in Green Color

I read somewhere in forum you should
give explanation if you are not using family name
what should I fill in this

And level for eduction for minor 2 year
put N / A right

This may be a problem. I recommed you to initate a seperate thread for this question so that it does not get lost along with other question and asks others in same boat to share their experiences.

Though completed unrelated, There was a recent notification from US Embassy for US visas only that people who do not have family name will not be issued Visas and they should get the passport amended first and then apply for US visa. Canada may or may not follow this but there are chances that they may
