It depends on where they are from. It is possible that the spouse can stay with you during the process depending on visa issues. For example, if the spouse if from a visa exempt country, they could come to Canada as a visitor and stay with you. You can then choose to apply through your spouses home country or in Canada. Applying through home country is usually faster to actually get PR but if an interview is needed, the spouse has to go back there for it. If you apply in Canada, work permit in about 6 months and PR in 12-18 months. If the spouse needs a visit visa and can't get one, you would have to apply through their home country or other country where they have residency.

After the spouse gets the PR visa, they can "land" in Canada and should have the PR card within a month or two.

The PR card does not give you the right to travel to the US. If you normally need a visa to go to the US, you would still need a visa.

It does not matter for sponsorship if you are a PR or a citizen except for one thing which is that a PR has to be in Canada to sponsor while a citizen can be living outside Canada as long as they can prove that they plan to move to Canada when the spouse gets PR.
