Few questions... Am I missing something obvious?

So,

In may 2014 we applied for spousal sponsorship outside of Canada and in August I received a letter I did not send in my IMM5406(additional family member). Okay, weird, so I resent it again and a few weeks later my WHOLE APPLICATION was returned to me... The letter did not say why or anything, I tried to follow up and the phone agents couldn't help. Whatever, I'll re-apply...

So I resent my application in September 2014 from inside of Canada and I finally, today, 7 months received an email from CIC saying they opened my application but... AGAIN! Imm5406... I am 100% sure I did not forget it a 3rd time... I am not understanding why... I called CIC and they just said to re-send it...

Am I missing something... I am using the 2013 version, the one you download from CIC.

cic.gc.ca/english/pdf/kits/forms/IMM5406E.pdf

Is the one I used. All names are in her native language and english. I am using a separate page since the addresses are to long... But I am not understand how this happened 3 times in a row...

Also

I need to re-send passport pictures since it took them longer then 6 month to open my application, what do I really need?

If you look at the document check list it says you need 9 photos,

but if you look at another check list it says you need 2 and some blank and some signed?.

Any info/experience? I do not want to screw this up... again, apparently

Ok you can't go back in the past but before I sent my application I photocopied every page. Not kidding I have every page that I sent to them. So if that happened to me I could look.

Also not sure but did you give them an email address? If they are sending you the document request by email then you can scan the document and send to them in email and say... Here it is, I have mailed out a hard copy on such and such date... Then they have it right away and you have proof in your outbox that you sent it. Whereas once the envelope leaves your hands how do you prove you sent it?

The other thing that I can think of is go on CIC website and make sure you have the newest version of the form. Read the form carefully and make sure you sign and date where you need to. I know that an old version of the form for family info had multiple places for signatures depending if you had kids or not. Don't think the new one does but read it like 5 times just to be sure.

If one says 9 and one says 2 I would send 9. But I think usually you need to follow the instructions on your country specific instructions, not on the generic one.

What country are you from?

You can never predict what CIC will pull out of their "hats.." -Sounds very strange and beyond frustrating. Be sure and put the English name(s) first and then the native. Sounds silly, I know, but they can be sticklers. Looks like you're using the latest version of IMM5406.

With regard to the photos - If I were you, I'd wait on an actual request for new photos. If your "Country Specific" checklist says 9 photos, then the number included should be 9. About the age of the photos; We took the photos 2 weeks before sending our application - One year later the photos were received in Sydney for processing (after she landed 6 weeks prior to that) and rejected for being too old... From all the experiences I read, it seems we are the only ones who have been told that they should be less than 6 months old when the card is approved for printing.

So, if this rule falls true for you, and you reprint them now, your process could very well take 6 months to landing... and if they wanted to, they could reject them then. btw, at that stage (as with us,) they requested only 2 photos - one dated (by the photographer) and one blank

Good luck with our CIC circus :)
