Hey Everyone,

I filled out IMM 5540 (Sponsor Questionnaire) a while back and I am now doing a thorough read through. As I look over the form and my answers, some of the questions seem vague (as I perceive them to be). I don't want to give the Visa Office any excuse to return my form to me and cause delays! :(

I wanted to get confirmation on a few of the questions from some senior/experience members on this forum or anyone who has successfully completed this form (IMM 5540).

RE: SPONSOR QUESTIONNAIRE

Question 4. Provide the following details of your father and mother
When it says family name for father and mother, I assume they are NOT asking for maiden name of my mother?

Question 10. Where you yourself sponsored to come to Canada? When?
Since I am a Naturalized Canadian, is this question asking for the date my father applied for sponsorship, the date he got sponsorship approved OR date we landed in Canada?

Question 11. Are you currently living in Canada? Since when?
I assume this pertains to arrival date in Canada? Just don't want to list same date as question 10 (above) if my answer is also based on arrival date!

Question 11b. Are you currently living in Canada? Your address in Canada?
I assume current address (this has been given in other documents)? Or are they asking for first address when I arrived to Canada for the first time?

Question 16. Have you ever submitted a sponsorship application for another person before?
I did a VISA - tourist visa with my girlfriend (now my wife) back in 2014 and it was rejected. Wanted to verify if this would be a form of "sponsorship" for a Tourist Visa? Someone mentioned otherwise.

No, sponsorship is sponsorship.
