few question regarding generic form!

I have a few questions regarding Generic Application Form(IMM0008)

1.Question 7 a) have you received your CSQ? What is this?

2. Question 12) previous country residence, I study in Canada for 3 years before, should I put it in this form? because I am currently living in Canada as a visitor, should I include my previous Canadian residence info or not?

1 - it relates to quebec applicants, so if you're not looking to move to quebec, say no or n/a

2 - yes - as studying in canada is a part of your personal history, and you indeed lived there for more than 6 mo. as a student, you would included that as a country of residence.
