Few doubts before I book upfront medical.

I am planning to book for my upfront medical test.

1) Ask a Pnael Physician on the list to do an Upfront Medical Exam
2) I only have my EE profile number ( ITA letter) & is it all the doctor needs to proceed with test ?
3) After test make sure I get - ''Information Printout Sheet'' (IPS)
4) The details on IPS should be for - # IME: Non EDE & # Upfront Medical type: Worker

Please confirm if the details above are correct.
