If I am adding Fingerprint card then do I need FD-258 form with fingerprints on it? I am bit unclear about it.

You can take a print out of the same on normal A4 paper and take finger prints on it If you go to a professional services for Finger printing they will have the same form FD-258 in a card format. FBI accepts both, the card and the A4 size paper
