Hi Trick, I don't remember sending any cover letter to the FBI. Just the form and the fingerprints. My memory may be playing tricks on me, and I could be wrong, but even the checklist does not mention anything about cover letter:

http://www.fbi.gov/about-us/cjis/background-checks/fbi-identification-record-request-checklist

For more information:

There is a cover letter. It's on the FBI website.

The cover letter is the application form.

Thanks everybody,

So as per Canadiangirl,Form is the cover letter.I have actually submitted FBI clearance long time ago but they have requested again.Last time I submitted I did not complete any form.

Yesterday,I went to the police station and I got my finger prints done on the card which was pretty much similar to the card which I submitted to the FBI for police clearance.

I am not sure if there are any changes as far as the card is concerned. When I look at the cover letter which is a form ,its point 6 says .
Current fingerprint card—no older than 18 months.

What does that mean?

Though the card on which I got the finger prints done had exactly the same information which I found in the form on the FBI site

Am I good to go with the card on which I got the fingerprints done.?Can somebody please answer.

Thanks,
Trick

I have one question regarding the form. While filling up I came across a field in which I have to mention my country of residence.

I am in US right now. However,I am an Indian citizen.Do I need to mention India or USA as a country of residence?

Where are you living right now? That is your country of residence. If you are not in India, that is not your country of residence. India is your country of nationality.
