When adding my family members should I also add my spouse who is also already in my application? Should I also add myself in his direct family members list? Seems repetitive.

No, you are adding family members that have not already been mentioned in the application, eg parents and siblings.

Are there missing family members for your child? If yes add them. If not, then nothing to add. Usually family members are parents and siblings etc..
