I am confused on what to put on the forms when it asks for my Family name. Only on one application did it ask specifically for the family name listed on my passport.

On my passport is my maiden name so on the forms do I list my new married name or my maiden name?
 
Complete all forms with the name currently on your passport, so they are consistent with each other. If you change your passport to your married name later on, you may submit updated copies. We did this, and it was perfectly fine.

When it asks other Family names that I have gone by I am assuming that I would put my new married name?
 
Charlie, when you say updated copies, do you mean copies of your passport or of the forms? I changed my last name and just sent them a copy of the new passport, but I'm not sure if that's sufficient or not.
