Family Information Form IMM5707 or IMM5645

Hi,
I am indian going to apply for study permit under non SDS. I am confused which form to attach for Family information. Since, my brother is pr in Canada should i mention it? Since there is no column of sibling in IMM5707, should i submit IMM5645 instead?

Hey which one did you use ? and how did you mention about siblings if you had chosen IMM5707 ?
