EXTENSION OF WORKING PERMIT..

Hello everyone,,Im a Live-in Caregiver too
Does anyone here could help me on how to extend my working permit which will expired on FEb 21,2013?,,,
Can i used my old contract or i need a new one to be sign?
what are the necessary requirement when applying extension of working permit?

Hi anybody could help me? I am wondering why i can not validate my form of extension form which the question is "what work permit im applying for" and answered as live in caregiver but doesnt accepted or validated why? Please help me thank you
