Extension of work-permit (post graduation)

Hello

I had a strange case regarding my post-graduation work-permit (PGWP):

Last year (Fall 2013) I was graduated (master), then applied to PGWP. My older passport expiration date is mid December 2014. Logically I should have been issued a PGWP up to this year December. However I was lucky enough to get one year extra!!!! The agent made a mistake and issued my PGWP up to mid December 2015 (probably a typo, 15 instead of 14).

Now I have renewed my passport (have a different PP No.) and have a valid PGWP up to Dec 2015 (but with an older PP No.). And according to my degree (two-year master), I am eligible to get a PGWP up to three years.

Question 1: Do I really need to apply to extend my PGWP before Dec 2014, because my PP has changed?
Question 2: If I do not apply to extend my PGWP before Dec 2014, will I be in trouble? In other words, should I be responsible for th CIC's mistake?

Appreciate your feedback.
