Extending visa past 6 months requires medical exam?

Sent off a spousal permanent residence application. Need to renew my visa very soon. No time to get an exam. Was planning just to get my visitors visa application extended one last time for a full year in order to deal with processing time and potential delays.

What should I do? I believe i can ask for a full year anyway without the exam if people think it's a good idea. Obviously, i shouldn't. I just don't want to have to go through this process an extra time if i don't have to. Is it possible to file my application as normal, ask for a full year and perform the medical exam at a later date?

File as normal. If a medical is required, it will be requested.

Ah, help! I did the survey and included that i would be extending my stay past 6 months and now this box showed up: "Proof of upfront medical exam (required)"
It would seem i need proof that i have an exam or have one scheduled. What are my options? If i attempt to complete the application and fulfil payment without a uploading a proper file indicating that i have had an exam or have one scheduled... i am not sure if it will affect my application in a bad way. All signs point to "Just wait for a request to have one done", but this box is stopping me.

Any further advice? Should i do a survey that doesn't include 6+ months to remove the requirement?

There is a question that asks if you are going to do a medical upfront. You answered Yes...
