Extending PGWP

I was entitled to 3 year PGWP but i only got 2 years because of my passport exiry date, recently i have renewed my passport and im ready to apply for my remaining one year. Can someone help me out a bit? What are the documents I need? i have scanned my new passport, old passport and current PGWP and all the stamps and visas i have from canadian entries. All i need to fill out is IMM 5710 and IMM 5556? and i also have a copy of my college diploma. Is there something else i am missing? Please someone help me out.

Online is really confusing and giving me trouble, so is it better to do it by mail? as i said what else am i missing? Thanks.

Looks good...include a cover letter in the pdf with your work permit explaining the situation..

Did you apply online or by mail? the new system is really confusing.

no I have not applied for the work permit in about an year...I would say apply online because it is a huge waiting period right now..try to be as truthful as possible...in the form I would say select initial or extension of your work permit and then add the cover letter...when I applied for work permit i did on the basis of transcript...CIC emailed me to upload the graduation letter from college as transcript does not show when did I graduate...there was only one slot of uploading and I added a cover letter as first page explaining that the transcript does show the graduation date and the transcript as the second page..

you should be fine as long as they get an idea as to what you want them to do....like if you enter your name differently than from your passport, they will themselves change your name...I am not saying fill the forms wrong but as I said try to be as truthful as possible....but if you lie about your status or anything major like that then is the trouble..
