There are couple questions: 1. Anyone know how to use positive LMIA to extend work permit?

2, Based on CIC, only employer can submit the application for extending work permit

3. I am a PGWP holder, I am qualified CEC and get LMIA from current job. Can I extend my open work permit if I got ITA?

Not quite right - only an employer can apply for an LMIA. If your employer gets the LMIA, then you can apply for a new work permit with the LMIA as a Temporary Foreign Worker.

??? You apply to to extend your work permit - if you apply with the LMIA, you will get a closed work permit, however.

If you get the ITA, you will be eligible to apply for the Bridging Open Work Permit after you submit your complete application (as long as your PGWP is still valid).

Be aware that there are 2 different types of LMIAs - one that can be used for both a new work permit AND the PR application (if your employer paid the $1000 fee), and one that can only be used to support PR applications (no fee).
