My employer will not provide me the job responsibilities. However my colleagues are ready to confirm them and get them notarised. What should be the format for this ?

The format would be pretty much the same, except for the initial sentences would be different, where the colleague mentions in a sentence or two who he is and how he is related to the applicant. In that case please note the following.
- When you are getting the letter from a colleague, ideally it should be at least someone senior to you, preferably from the same department (as this can probably help you avoid any issues)
- If the person who is confirming your roles and responsibilities is your Reporting Manager, you can retain that field in the letter, otherwise you can remove it. Also, please attach his visiting card/contact card as part of the letter.
- As a policy, generally salary is not disclosed between colleagues (as some companies have that mentioned in their employment contract), so to be on the safer side, I would avoid the Annual Salary part in the letter, instead submit copies of the employment contract and pay slips as proof of pay.

Unfortunately I don't think my manager would be ready to give that and there are no other senior members in my team.I would be get all other details on letterhead except roles and responsibilities. Would it do if I get it from my colleague?

I think it should be fine. But, I would suggest you go for a second opinion from experienced members on this forum.
