Error on Medical Form. it states Panama not Philippines

Hi everyone!

A few years ago my common law partner and now fiance immigrated to Canada from The Philippines as a permanent resident. All has been great.

We are now ready to bring her daughter here. I do remember when we first applied for my partner, even her daughter had to take the medical test even though she wasn't joining her at the time.

We decided to get her daughter a new 2nd medical test anyway. We got the paperwork and also the doctors office said they sent this paperwork to the Canadian Embassy in Manila.

We have not applied yet but we about to when we noticed on the medical form it says the test got taken in PANAMA and not Philippines. We are guessing the person making the papers had a dropdown box in the countries and simply chose the wrong one.

We called and email the doctors office for many days and hours, never an answer. Finally after 10 days we got a response saying they can't change it because it was sent to the embassy already.

We can't easily send her daughter to do another medical test as it's a 5 hour bus trip and the family is not very competent. We always had to do it when we were there.

Is there any way to submit the medical documents that say Panama on them with a letter noting their was an error. I mean it is obvious that if they were sent to The Philippines Embassy that it would not have come from Panama. But then again I'm not so sure how strict they are with those details.

Any ideas?
