Error in medical request form

Hi,
I have received a request from CIC to undergo medicals. Barcode stickers, fee receipt and some instructions for mode of communication are also attached.

I found an error in the medical form. My marital status is incorrectly reported as "single" instead of "married". My wife's marital status is correctly indicated as "married".

Question -
Should I ask CIC to send a corrected medical form? Is it a serious gap if this form has error like this?

I am worried about the delay that this might introduce.

Please advise.

Cross it out and write "married". It's a clerical error. They obviously know you are married as they sent your wife's medical request too.

Yep. A medical is a medical, married or not. It wouldn't hurt for you to send them a fax stating the error that they made on the form and that you've corrected it and completed the medical.
