Employment reference letter affidavit format India

Dear experts,
Can someone please share the employment reference letter affidavit format..
Iam working in a same organisation from 10 years and can't go to HR to get them.. requested on of the colleague to sign the affidavit.. need to get the format to complete. Please help...

First of all know whether affidavit will work or not. Ask direct SINP
