emedical tracking sheet

Dear All,

i just had my medical exam yesterday and they gave me a emedical tracking sheet. The staff told me to submit this to the office who is processing our application/visa. I have informed my wife as she is the one who applied for me and our daughter. She has no idea where to submit or Upload the Emedical Tracking Sheet.

If anyone have an idea on where to upload or submit the Emedical Tracking Sheet, kindly share the information.
We have checked in google and we cannot find any answers.
