On the checklist (IMM 5491), it asks for Proof of medical examination IMM 1017. Can eMedical replace it? Or CIC will require both (IMM 1017 and eMedical)?
 
Your proof will be the receipt from the doctor.

On the instruction, it asks the applicant to bring the IMM 1017, which we forgot. And the doctor's office didn't ask about it when we did the medical examination. The "receipt" we got is a eMedical form.

My question is do I still need to go to the doctor's office and get him to sign the IMM 1017?
 
No, you don't need to go back to the doctor's office. The form you received is what you need to include.
