I have received a letter to show documents that I was eligible for ohip for the past few years. I showed them documents and they said it was not sufficient documentation for some years and they suspended my card. If I show them more proof that I was in Ontario for the past 3 months would that restart my coverage? or am I doomed if I can not gather old documents from the prior years?
 
If you were using your card during those four years - then showing you have been in Ontario for the last three months won't be enough. They are looking for proof that you qualified for OHIP during the period you were using it.

Did you leave Canada for any long periods during the past few years?

Did you get all your entry/exit records from Canada and other countries you visited during this time? Or passport stamps?
