Hello,

do you have to write N/A on all blank fields? Some of the fields are disabled when you fill it on computer. Do I have to write N/A with pen after they are printed?

No, you don't. The fact that they are disabled and greyed out shows that they are not applicable to you.

Anything that you write on the forms in pen will obviously not be encoded in the bar codes. Therefore, it may not be entered in the GCMS system.
