ECA from MCC: has anyone from Bangladesh recently done it?

Hi, I'm Dr. Samina. I have an MBBS degree from DU in 2013 and my medical college is Holy Family. I passed FCPS surgery Part 1 in 2015. I've been in training ever since. I need to know what are the documents I have to submit to MCC for ECA. And what should I choose as my NOC.

If anyone knows, please help.
