Earliest stage to take Medicals and shorten WP process?

If an Indian applicant has a job offer and 2 year LMO, what is the earliest that he can take medicals so it shortens overall Work Permit application time? The obvious downside is possibility of paying for medicals and being denied WP, but is willing to take that chance. I am very new to this visa stream so please forgive what may have an obvious answer - does applicant have to apply for TWO visas TRV and WP, or does the WP application automatically cover TRV? Am thinking I read somewhere that you only pay for a WP. Thanks for your advice.

The outside Canada application package includes WP/visa application. The applicant can submit medicals together with the application if you can find and convince the DMP to do the medicals without the letter from the visa office/CHC/embassy.

Note that different visa offices require different documents as attachments so check the website of the CHC you are applying frm.
