Do you have a new birth certificate with your current name and the name of your adoptive parents? Does it still say that you were born in Canada? I am not a lawyer but I have the idea that once you are adopted, your adoptive parents are your parents and your biological parents are legally speaking not your parents any more. However, as you were born in Canada, regardless of who your parents are considered to be, you must actually be Canadian.

You should contact the nearest Canadian embassy to where you live. You can send them copies of everything you have and ask their advice. If your birth certificate is considered proof of your citizenship, you can apply for a Canadian passport through the mail. If not, you can apply for a citizenship card through the embassy and once you have it, you can apply for a passport.

You can find contact information for the embassies here: http://www.canadainternational.gc.ca/washington/offices-bureaux/index.aspx and Passport Canada here: http://www.ppt.gc.ca/index.aspx
