Doubts regarding medicals

1. I go to doctor and ask them i need upfront medicals for EE/Permanet Residency ?

2. Am applying under FSW/EE. So whats the correct category ?

3.After tests are done, the doc will give IMM 1017B Upfront Medical Report form to me which i upload to MYCIC ?

Hi nokia3315, have you received ITA yet? If you have, you should ask a doctor to do an Upfront Medical Exam, as mostly they are unaware of Express Entry. The category should be Worker and you shouldn't leave the appointment without the "Information Printout Sheet", that's what you are going to upload to you your application in MyCIC.

Yes make sure you get the information sheet which you will have to upload post ITA
