Hello,

I landed in Toronto before (2009), but needed to leave later although I did back and forth some occasions. However, now I am finally able to go back, this time Ottawa. I would like to apply for OHIP. I have my permanent residence card, national health insurance card, driver license valid. According to the settlement.org website, Canadian Immigration Identification card, Permanent Resident Card,
Confirmation of Permanent Residence (IMM5292), Record of Landing (IMM1000) are needed for OHIP. I have IMM5292, Permanent resident Card.

I am grateful if you could give me advice on the following questions.
1. Are my passport and permanent resident card enough to enter Canada as permanent resident?
2. Is IMM1000 passport stamp? If not what is like? It seems I do not have it.
3. Is Canadian Immigration Identification card same as permanent resident card? I have permanent resident card.

I hope I can arrange these documents, if they are different from what I have at the moment, once arriving in Ottawa soon.

2. If you have IMM5292, you would not have IMM1000 because the former replace the latter.
