Documents needed for applying for work permit after receiving LMO

Dear All,

By Feb 2013 , my post graduate work permit gets over.
Employer applied for a LMO and i received one as a Business and Operations manager.
I am planning to apply for my work permit myselflease advise
1. what all supporting documents i need to submit
2.my wife holds a spousal open work permit.I need to apply for her as well
3.How long it might take

Please advise me on the documents that needed to be submitted.
