Documents for spouse open workpermit medical exam?

Hi,
I am preapring an application for my spouse for open work permit outside canada. The hospital is asking for embassy documents for medical exam. I am not able to figure out what are the embassy documents they are asking for? I asked them but they were not sure about it as well. Please help.
