Documents for EE profile clarification

Hi, I need some clarification regarding documents required.

1. The employment letter. Does it just state the facts or is it also a refrence/ performance letter where my employer has to specify how I performed.

2. Proof of education. Do I need a copy of my degree of just a letter from the university (Canadian) stating that I have graduated from there.

3. Is an up front medical better or should I wait for when I get an ITA.

4. Proof of funds. If I have credit card payments due, would that be a problem at the time of showing proof of funds?

5. Do I need to attach a copy of my old passport which has current US visa, old Canadian visas or just the copy of a new passport.

6. What is the template for a resume.

1. No performance evaluation required.

2. Copy of degree / diploma is mandatory

3. Wait for ITA

4. No, as long as you have the required amount

5. You only need a copy of the new passport's biodata page

6. Resume is not needed for EE
