Documentation error!

Hi Seniors,

In the refusal letter..

"You have submitted documentation which lacks authenticity as part of your application. This has diminished your overall credibility of your submission"

What that actually means?

This can even happen of spelling mistakes, submitting a duplicate document without attesting. right?
