For the sponsored person and family members:

Proof of medical examination for the sponsored person and each of his/her dependents (Copy 1 of Medical Report: Client Biodata and Summary (IMM 1017), signed and provided by the panel physician )


Does it mean the physician will give the sponsored person a copy of the form IMM 1017 Client Biodata and Summary, after the medical exam, then the sponsored person will have to send this form to CIC?

Yes. The doctor gives the applicant a document which is the first page of the doctor's report. The applicant submits that with the rest of his/her application to Mississauga (assumning that it is an outland application). The rest of the report is sent by the doctor electronically to Ottawa, and it is extracted from there electronically by the Visa Officer who examines the application. I am unsure whether the first page is still referred to as IMM 1017. I do not recollect that detail appearing on the page I was handed after my medical. What I do remember is that it was not a yellow document (ie the document which the applicant had once upon a time to take with him to the medical).

If you google CIC medical exam for permanent residents you will see it all set out. In its announcement CIC seem to have overlooked the requirement that you must bring funds to pay the physician his fees; save for that the list seems complete.
