Document List for Spouse Open Work Permit

Dear Forum Member,

I am in Canada on ICT work permit and my wife having Visitor visa with visitor record with same validity as my work permit.

I am applying her Open work permit inside canada and would like to know any additional documents(checklist) required apart from forms (imm5475e).

Please reply with all the documents I can attach while submitting my wife's application.

Documents listed on the checklist are all you need.

Thanks Andy, May I know which checklist you are referring here?, Also for UCI column in the form should I left it blank or I need to fill the UCI of Visitor Record

Yes, your UCI no. will never change so you can fill that in unless it says "for office use only" or something on those lines.

Thanks Andy for your quick response, one more question Do we need to upload the signed document? like Validate--> print --> Sign - -> Scan the signed one and upload.
