Do I need to get the medical exam done in the same state or country?

I am in state A. The medical exam center in State B is nearer to me than it is in my state (state A.) Is it mandatory to get it done in the same state that I am residing in or do I have the option of getting it done anywhere in the country?

Anywhere in the world as long as the physician is recognized by CIC will work.
