DIFFERENT NAME ON CERTIFICATES THAN PASSPORT

hi, friend .
i have my OLD name on college certificates is without surname , but my passport which i got in 2012 is with new name including my surname.
now i applied for mpnp, and i received LAA to apply full application,, will it be problem for me ?.
or i need to provide any proof of name change,
my passport is with new name, all college certificates before 2012 are with old name .

what document i should upload to mpnp for this .

please guide me .

This is a common issue for most Indian applicants. This does not fall under "name change after marriage" category for which CIC and Provinces require more documentation.

You can provide either a Legal Affidavit in which you declare all your names and state under oath, that all of these belong to you and refer to the same person. Or you can just provide a letter of explanation where you declare by yourselves (not under oath before a notary) that all of these names belong to you. The former will be more stronger than the latter.

thank you sir for your helping reply.
today i prepared a affidavit of one and same person , stating that i added my surname to my name in 2012 and both names belong to one person only. this affidavit is from the side of my father.
it that ok .
