Did someone been asked Long-Form Birth Certificate

Had CHC asked someone to submit LONG-FORM BIRTH CERTIFICATE?

What is it?

Yes, everyone gets it (most, I'd say) from the CHC alongwith the Meds.
It's just ur birth certificate, which u get from the hosp.

You know the days when myself and spouse were born, it was mid-wives delivering babies at homes... There is no such certificates etc...

I am however searching for kids hospital Birth Notification Certificates...

Shall I write an explanation about myself & spouse? or what people does in this case...

Same was my case too.

For mine & my spouse's, I sent the Secondary School Cert, highlighting the DOB, that's what is accepted in most parts of the world alongwith what's written in our passports. It was accepted by CHC.

For my kids, we had the b/certs.

Did you sumbit your birth certfi...original one from where all the birth certfi..will be issue.becuase iwe having difficulties to sumbit my partner birth certfi....can we show only school certifi..is that work..

We did submitted Notarized Copies of Birth Certificates for all of us... My all kids are born in UAE expect one daughter in Pakistan. CHC is not convinced about her birth in Pakistan... They even asked to submit passport showing that my wife did went to pakistan to deliver baby and also Long-Form Birth Cert.

Strange but we have to provide what has been asked...

In my case I have my DOB mentioned on the Secondary School leaving Certificate. Is it the same to your Secondary School Certificate?
