Dear Raaju,

You need to attach your Birth Certificate as date of birth proof. It is clearly mentioned in the application guideline.

Birth certificate is mandatory.

I submitted the same, and it worked for me.
I created a cover letter mentioning that I don’t have a birth certificate and sending following document in support of that.

Dear NSG, I also did the same but my SSC certificate does not have father's name. DO you think it will be accepted?

No, for it to substitute a birth certificate it has to have both parents' name. You should have sent an affidavit of your parents as a birth certificate replacement.

I have sent an affidavit, it has my father's name but not mother's name. My matriculation certificate doesn't have father's name as it is from ICSE board.

I have dispatched my application today under NOC 1111. If you think I should change the affidavit then I can recall the courier and send it again tomorrow.

So, you have the SSC certificate with only mother's name and an affidavit with only the father's name. I don't know of any case where this has been done to tell you if this works. This might work but it might not. If you want to be absolutely sure and not lose sleep over it while waiting, I think it is a good idea to get the package back and send with it an affidavit signed by both parents with your DOB. Also include a cover letter explaining what you have for DOB proof instead of a birth certificate. Since you are 1111, there is no need to worry about the cap.

I have submitted SSLC certificate with parents name and DOB, it was accepted by CIC.

I have recalled my courier and will get a new affidavit made tomorrow which I will send with a letter explaining that the board from which I did my matriculation does not print father's name on the marksheet.

Can you not make an affidavit with both parents' names and signature? That's the best way to do it.

Yes, I intend to do that. Any other suggestion? Shall I attach any other document like PAN CARD etc just to show that my DOB is same everywhere?

That would be enough. Just send the affidavit and a cover letter explaining the purpose of the affidavit.
