hi everyone,
im new to this forum,i was hoping somebody could answer me or have the same situation with me might give me some inputs on what to do.
1.i have 2 kids ,my daughter is 7 years old and living with me(my parents) and she had my family name since she was illegetimate,her dad did not acknowledge her that time,and my son is 5 years old and currently living with there dad(my ex)but ther dad acknowledge him and had his family name.
2,we are not married we separated long time ago.
3.im starting to fill out forms now and in the form stated "custody papers",do i need to provide that even he will willing to give me my son ?
4,is there additional form or requirements that i need to fill out in the future?

Your children have the same father? If not and if the bio-dad of your youngest is approachable, just have him sign the IMM 5604 and get a valid identification (ID) from him showing his photo and signature.

About your daughter, if there is no mention of the bio-dad's name on her birth certificate, just execute an Affidavit of Illegitimacy and Sole Parental Authority.

yes they have the same father,and his not on her bith certificate as bio'dad..
,regardlwss with there dad were in good terms now for the sake of the kids,but were not back togethwr though.
