Delhi -- how to fill out phone number? area code?

Hi,
what should we write for phone number for INDIA

country code- 91
area code-11 (for delhi)
phone number- 99_________(cellphone number)
but when the number is dialled from canada..the area code etc is not needed...
this is how i dial 011(to get out of canada)-91(country code for india) - cellphone number

should we leave the area code blank?

You can include both or include an additional document metioning how you can be contacted in which you can mention the step you have detailed

Either leave the area code blank, or put two numbers down - one with everything; and one as you would actually dial to get connected from Canada, with a note to that effect.
