DECLARATION FROM NON- ACCOMPANYING PARENT FOR MINORS

hello to all, I am the PA (wife) with 2 dependents in the application . I am currently filling up forms for my application. From the checklist supporting docs number 3 it says i must complete declaration from non accompanying parent for each child if the sponsor is not the other legal parent of my children. my problem is i could not open empty form after filling up one form. I need one more form to fill up since there are 2 dependents. Can any one help me with this? thanks in advance

On my app, i have 2 dependent children. My husband ended up printing two empty forms,writing the names of children by hand, and getting the other parent to sign in front of justice of peace. Also you need a photocopy of the parent ID..but if you cant get it like in my case..he wrote a letter indicating the reason why..ie. parent giving a hard time wont give permission for photocopy.
