Date of Birth Issue - Application Returned - Urgent

Hello Everyone,

My application got returned with the following letter from CIC.

" PLEASE NOTE, WE DO NOT ACCEPT PASSPORTS AS PROOF OF BIRTH, THEREFORE WE ARE UNABLE TO
ACCEPT DETAILS FROM PASSPORT. WE DO ACCEPT SECONDARY SCHOOL RECORDS THAT CONTAIN THE

FOLLOWING:

1) APPL|CANT'S NAME

2) DATE OF BIRTH
3) NAME OF PARENT/PARENTS

IF YOU ARE NOT ABLE TO PROVIDE A BIRTH CERTIFICATE WITH ALL 3 CRITERIA, WE REQUIRE AN
AFFIDAVIT STATING ALL OF THIS INFORMATION OR THAT A BIRTH CERTIFICATE CANNOT BE ATTAINED. "

I sent Certifcate of DoB from Indian Consulate and my College leaving certificate. The problem is that the name on my leaving certificate is B. Anil Kumar , but on my passport it is Anil Kumar Banoth.

They have asked me to submit an affidavit. Could you please guys help me with the format of the affidavit ? Or is there any other way out ?

3. I am providing this affidavit in order to verify my birth details, as I am unable to attain birth certificate from the relevant issuing authority and I am attaching notarized copy of my 10th Class (High School) Certificate and Passport as supporting documents with this affidavit.

And I make this solemn Declaration conscientiously believing it to be true, and
knowing that it is of the same force and effect as if made under oath.

you can go to any lawyer and get the above notarized.. they will take around 20-25$

Birth certificate is on the document checklist. Under point 11. Identity and Civil Status Documents.

I have got my DOB affidavit from India and it has been signed by my father. Does this affidavit needs to be signed by both Mother and Father or signed by anyone would be fine ?

Hi Bhopali,

As far as I know, It should be the individual who sign the document for affidavit. I signed mine and it was ok because I made the declaration.. Please keep a copy of high school certificate for proof also.

Only thing is, even in my 10th Class Marks certificate, my name is B. Anil Kumar and NOT (Anil Kumar Banoth). Will that be a problem again ? Any workarounds ?

I don't think that will make any difference.. it should be ok.. mine was also similar case...

As per this post we can submit secondary school certificate which have these details.
Please let me know if we need to submit any affidavit also in this case or just this certificate with a cover letter will work. Please suggest.

1. Last time I have paid by using online payment option, my question is do I need to pay once more or I can use the same receipt and send the same copy once again?
2. Now I am back to my home country, so would I need to provide the PCC from Canada as well?
3. Also since I am back to my home country, so in form IMM0008 Question 13 will have my home country detail and now I have to provide canada details in question 15. Please clarify.
4. IMM5669 Question 6 point d, I believe I will have to choose option "yes" here. What shall I be writing under the reason for the same? Please advice.
5. Now, Do I need to get my Indian PCC once again?
