CRA tax return/assessment - for dependent sponsorship

I am using the checklist 5491E for family sponsorship and I see that it is asking for a CRA assessment, and asks to explain why if it is not available. I was going to attach my last US tax return and the last 3 months' payslips in Canada. Will those be sufficient to get approved as a sponsor?

If there is any specific format for the explanation letter, kindly point me to one.

U.S tax return won't replace the Canadian tax return , last 3 months pay slip will help prove income even though there is no minimum income required for spousal sponsorship.
if you can't supply your Canadian tax return explain what prevented you from doing so .

a normal letter with your name and info , indicate the purpose of the letter , the form # and question # and explain yourself , make it short and to the point .
