Documents list for addition of new born baby girl

2. imm 5406 me and my wife
3. passport copy for baby
4. birth certificat
5. resident permit for saudi arabia for all family with new baby
6. 150 CAD bank draft
7. four number of photos
does i have to send any covering letter or just file number on the envalop.
plz advice or any sample for letter, soi can follow
