Country of issue is India. Place of issue is the actual city where it was issued. Could be Ottawa / Toronto / Timbuktu.

This is the only correct answer.

Country of issue is the country that issued the passport; in this case, India.

Place of issue is the specific office where the passport was processed; in this case the Indian consulate in Toronto / Indian embassy in Ottawa, or whatever is listed in the passport.

(Also: embassies are only found in country capitals, so there cannot be an Indian embassy in Toronto.)

Hello all, I entered wrong country of issue , i entered canada instead of india, what should i do now ? Please help?

That's probably a very common mistake IRCC agents see every single day.
Don't worry about it. Just send a webform with the correction (or if you didn't get the AOR yet, do it when you get it).
